Probation Service errors allowed a man with a history of violence to stay free and murder Zara Aleena in London in June

Jordan McSweeney was considered a medium not high risk – meaning when he breached his licence his return to prison wasn’t treated urgently enough.

Significant delays in allocating a probation officer also meant a proper risk assessment wasn’t carried out before he was allowed into the community.
