Village Surgery in the High Street is to shut its doors following a poor inspection report from the Care Quality Commission.
Inspectors rated the safety and effectiveness of the surgery inadequate.
They noted the practice was not monitoring all patients on high risk prescription medicines as required.
Medication reviews were also not always completed or fully noted or recorded.
Staff had also not been completing weekly asymptomatic COVID tests.
Patients registered there will be moved to other surgeries.
