The Acute Emergency Department Frailty Team, and the Kingston and Richmond Proactive Anticipatory Care Programme Team are up for the national NHS Parliamentary Awards.
The ED team works to make sure that elderly patients are fully assessed and safe to be discharged.
Meanwhile the PAC team has been working to manage increased demand to healthcare.
